Customizing Guest Forms

Last updated March 11, 2026

Guest forms collect information from guests before their trip. Customize what you ask in Settings > Guest Forms.

Two Types of Form Fields

Built-in Fields

Hookset includes 6 built-in fields with three visibility modes:

| Field | Description | |-------|-------------| | Phone | Guest's phone number | | Skill level | Fishing experience level | | Preferred fishing method | Fly fishing, spin casting, etc. | | Wader size | For lodges that provide waders | | Shoe size | For lodges that provide wading boots | | Dietary notes | Food allergies and preferences |

Each built-in field can be set to:

  • Required -- Guest must fill it out to submit the form.
  • Optional -- Field appears but can be left blank.
  • Hidden -- Field does not appear on the form.

Tip: Only require fields you actually need. Shorter forms get higher completion rates.

Custom Questions

Add your own questions to collect lodge-specific information. See Custom Guest Form Questions for details on the 6 question types available.

Form Preview

Click the Preview tab to see exactly what guests will see when they fill out the form. The preview shows both built-in fields and custom questions in their current order.

Applies To Setting

Each custom question has an Applies To setting that controls where it appears:

  • Join form -- Shown when a guest joins a party via invite link.
  • Trip prep -- Shown on the trip preparation form before arrival.
  • Both -- Shown on both forms.

Built-in fields always appear on the trip prep form. Their visibility on the join form depends on the field.

Reordering Questions

Drag and drop custom questions to change their display order on the form. Built-in fields always appear above custom questions.
